AST on a Blood Test: What It Adds Beyond ALT
AST is not a liver-specific enzyme, which is why it is read next to ALT rather than alone. What the ratio between them does and does not predict.

What does AST measure?
AST is an aminotransferase released when cells are damaged, and it leaks into serum the same way ALT does. The difference is location. ALT is concentrated in liver cells. AST is present in liver, but also in heart muscle, skeletal muscle, kidney, and red blood cells.

That makes AST the less specific of the two. A raised AST tells you cells somewhere are turning over faster than usual — it does not tell you they were liver cells. This is why the pairing matters: an isolated AST rise with a normal ALT points away from the liver rather than towards it.
Why is AST always read next to ALT?
Because the relationship between them carries information neither gives alone. The AST/ALT ratio has a name — the De Ritis ratio, first described in 1957 — and it is used clinically as a pattern-recognition tool.
The intuition is straightforward. Because AST has non-hepatic sources and a different clearance profile, conditions that damage the liver in different ways shift the two enzymes by different amounts. A ratio well above or below the typical range is a hint about mechanism.
In a 2023 cohort of patients whose AST and ALT both sat within the healthy range, the median De Ritis ratio was 0.94, with a 5th-95th percentile spread of 0.61 to 1.41.
That distribution is worth holding onto, because it is the context most routine results fall in.
Does the AST/ALT ratio predict anything on a normal panel?
This is where the evidence is weaker than the enthusiasm, and it is worth being precise.
A study published in the Journal of Clinical Medicine in 2023 examined 3,392 patients with stable coronary artery disease, of whom 1,697 had both aminotransferases inside the healthy range. Within that healthy-range group, a higher De Ritis ratio was associated with all-cause mortality at a hazard ratio of 1.16 (95% CI 0.94-1.42) per unit — which was not statistically significant (p = 0.159).
The confidence interval crosses 1. On this evidence, the ratio does not demonstrably predict mortality in people whose enzymes are already normal.
Where the ratio does carry prognostic weight is in acutely ill or specifically diseased populations — trauma admissions, certain cancers — which is a different question from what a routine panel in a healthy adult is answering. Findings from a trauma cohort do not transfer to an annual physical, and presenting them as though they do is the most common way this marker gets oversold.
What that means in practice
Treat the De Ritis ratio on a normal panel as descriptive rather than predictive. It is a useful pattern when something is already abnormal. It is not a validated longevity metric, and it does not belong in the same tier as the markers with population-scale outcome data behind them — see what is actually worth tracking.
What about the reference range itself?
The same problem that affects ALT affects AST: laboratory reference ranges are derived from local populations, and those populations were not screened for metabolic health.
The analysis that documented this for ALT found upper limits varying from 35 to 79 U/L for men across eleven laboratories, and identified the cause as reference cohorts that excluded known disease but not overweight and obesity. Since fatty liver raises aminotransferases, the reference populations contained undiagnosed liver disease, which pushed the upper limits up.
The practical consequence is the same for both enzymes. “Within normal limits” describes where you sit relative to that laboratory’s population, not relative to health.

It depends on the laboratory. Reference ranges are built from local populations, and the analysis of this problem found more than a twofold spread across labs for the companion enzyme ALT. Read your result against the range printed on your own report, and treat that range as a population descriptor rather than a health threshold.
What does a high AST mean?
That cells containing AST are releasing it. Because AST is present in heart, skeletal muscle, kidney, and red blood cells as well as liver, a raised AST is not specific to liver injury. It is interpreted alongside ALT and the clinical picture.

Can exercise raise AST?
Yes. Skeletal muscle is a significant AST source, so intense or unaccustomed exercise can raise it — and can raise it more than ALT, shifting the ratio. Testing several days after hard training avoids reading a muscle signal as a liver one.
Is a high AST/ALT ratio dangerous?
On a panel where both enzymes are already normal, the evidence does not support treating it as a warning: the association with mortality in that group was not statistically significant. A high ratio alongside abnormal enzymes is a different situation and one for a clinician to interpret.
The takeaway
AST earns its place on the panel by being less liver-specific than ALT, not more. Read alone it is ambiguous; read next to ALT it helps distinguish liver from muscle and one liver process from another.
The De Ritis ratio is genuine clinical shorthand with a long history, and it has real prognostic value in acutely ill populations. What it does not have is demonstrated predictive value on a normal panel — the study that looked specifically at that question returned a non-significant result, and that is worth knowing before treating the ratio as a personal health metric.
