This is one of the most common questions on any GLP-1, and the good news is that the drug labels answer it directly. The rule always comes down to the same thing: how many days are left until your next scheduled dose? That number, not guilt or guesswork, decides whether you take the missed dose now or skip it.

The catch is that each medication has its own window, so the right answer depends on which drug you’re on. Below are the exact instructions from each manufacturer’s official guidance. The quick answer, by drug

Here is the missed-dose window for each of the four common weekly GLP-1s. “Take it” means take the missed dose as soon as you remember; “skip it” means leave it and resume on your normal day.

Whether you take a missed dose depends on how many days remain until your next scheduled one; each drug has its own window.

Notice that Wegovy is framed by how much time is left until the next dose, while Ozempic, Mounjaro, and Zepbound are framed by how much time has passed since the missed dose. They get at the same idea (don’t take a make-up dose too close to your next one), but it’s worth reading your own drug’s rule carefully.

Semaglutide: Wegovy and Ozempic

Both Wegovy and Ozempic are semaglutide, but they carry slightly different missed-dose language.

For Wegovy, the guidance is: if you miss a dose and your next scheduled dose is more than 2 days (48 hours) away, take the missed dose as soon as you remember. If your next dose is less than 2 days away, skip the missed dose and take your next one on your regular day.

For Ozempic, take the missed dose as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip it and take your next dose on the regularly scheduled day.

The two differ because the products were labeled separately, and Ozempic’s guidance gives a wider catch-up window. Follow the instruction for the pen actually in your hand.

Tirzepatide: Mounjaro and Zepbound

Tirzepatide is simpler because both brands share the same rule. Take the missed dose as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip it and take your next dose on the regularly scheduled day.

So if your shot day is Sunday and you remember on Wednesday (three days later), you’re still inside the window: take it, then go back to Sunday next week. Remember on Friday (five days later)? Skip it and just take your normal Sunday dose.

Never “double up” to catch up

Across every one of these drugs, the same warning applies: do not take two doses close together to make up for a missed one. GLP-1 side effects (nausea, vomiting, reduced appetite) are dose-related and worst when levels rise quickly. Stacking two doses spikes your exposure and is a reliable way to trigger a rough day (or worse). The window rules exist to keep two doses from landing too close together.

If you’re changing your injection day on purpose rather than recovering from a miss, the spacing rules still apply: semaglutide should have at least 2 days (48 hours) between doses, and tirzepatide at least 3 days (72 hours), before your first dose on the new day.

What if you missed two or more weeks?

A single missed week is a minor blip. A longer gap is different, because your tolerance to the drug fades when you’re off it, the same reason the schedule ramps up slowly in the first place.

Wegovy’s guidance is explicit: if you miss doses for 2 or more weeks, either take your next dose on the regularly scheduled day or call your healthcare professional to talk about how to restart. Putting it together

A missed dose is common and, handled correctly, not a big deal. The whole decision hinges on timing:

  • Wegovy: take it if your next dose is more than 2 days out; otherwise skip.
  • Ozempic: take it within 5 days; otherwise skip.
  • Mounjaro / Zepbound: take it within 4 days; otherwise skip.
  • Any drug: never double up, and never take two doses inside the minimum spacing window.
  • Two-plus weeks off: call your prescriber before restarting.
